2013: Manchester United 2-1 Swansea City

Man Utd collected the Premier League trophy for the 13th time as they marked manager Sir Alex Ferguson's final match at Old Trafford with victory over Swansea City.

After goals from Chicharito and Michu made the score 1-1, Rio Ferdinand slammed home the winner from six yards out with three minutes to go, his first league goal in five years.

It was Ferguson's 528th and final victory of an extraordinary Premier League career.

The result put United 13 points clear of Manchester City, while Swansea were ninth on 46 points.
