George Byers and Jordan Garrick struck in the final five minutes of the first period.
On 89 minutes substitute Kenji Gorre sealed Swansea's fifth league win in six matches.
The Swans are now fifth in Division 1, two points behind fourth-placed Everton. The Hammers are one place and three points below them.
