The 17-year-old Dutchman fired the Blues a into ninth-minute lead and doubled the advantage seven minutes later after a brilliant turn for his sixth goal in eight matches.
The player signed from Ajax last summer completed his treble with a 70th-minute header from a corner.
The win lifts Chelsea to fifth, four points behind Liverpool, while Derby remain two points above the relegation places.
