The Premier League's flagship good cause programme, Creating Chances Places for Players showcases the vast amount of work that the 20 Barclays Premier League clubs undertake in their local communities.

Since the beginning of July we've been asking not-for-profit organisations to apply to 'win' Premier League players to raise awareness of the work that they do in their community.

Not only have you been a busy bunch - the entries have poured in from all across the country - but you have also been imaginative too.

The entries are currently being collated and shortlists drawn up for every club to choose one winning organisation who will receive a player visit and a £4,000 donation.

Every club will also pick five additional runners-up will also receive a £4,000 donation from the Premier League.
