Fulham

Fulham will engage men in Lambeth through a series of football, sport and gym-based activity sessions and then use Health Trainers and Nutritional advisors to signpost and refer the participants to the relevant health services. Specific attention will be given to the three main health issues in the area: sexual health, substance misuse and nutrition. Shop4Jobs workshops will also be delivered to improve employability skills;

Blackburn Rovers

Blackburn Rovers will target men of South Asian heritage in an area where residents are 45 per cent more likely to die before reaching 75 than the national average, 95 per cent more likely to die from lung cancer and 67 per cent more likely to die from heart disease. The club will use football and badminton to engage the men and then promote a health model of excellence leading to improved awareness of healthy lifestyles and cost effective and efficient delivery of health services;

Tottenham Hotspur

Tottenham Hotspur will go into schools and target fathers of children in Enfield - in the bottom eight per cent most income deprived in the country and bottom 15 per cent for unemployment. The project will promote knowledge of nutrition, oral health and physical activity and involve cooking sessions so the dads can put their learning into practice and improve their social skills and improve family cohesion.

Bolton Wanderers

Bolton Wanderers will target men who may be unemployed, on low-income or young and ex-offenders. They will also seek to help men with mental health problems. Bolton has high levels of unemployment which can often lead to mental illess.

A Health Trainer based within the club will work with the local PCT, Neighbourhood Renewal and Youth Offending Teams to target men who fall into these categories and look to engage them with football, cricket and boxing coaching sessions. These will help not only build the general fitness of the men involved, but also help develop other important skills like teamwork and communication.

Chelsea

Chelsea will concentrate its efforts on the North Fulham area close to its Stamford Bridge ground. They will work with their PCT to seek to involve local men in football sessions by setting up specific leagues and competitions in the area and encourage them to take their level one and two coaching qualifications. The scheme will look to improve the fitness of the participants, help improve their employment opportunities and give the PCT a chance to get across health messages to them.

Everton

Everton will work in some of the most deprived wards in Liverpool. Participants will be sourced not only from other projects that the club works on but also from referrals from other health agencies in the city. The project will also host match day events, which will be themed around the key health messages - giving the project the opportunity to access men on a mass scale. On match days there will be MOT Health Testing for male fans - offering the chance to educate men about different components of their health.

Portsmouth

Portsmouth will use their Health Trainer to target men and the issues of sensible drinking, promoting a healthy and active life, tackling obesity and smoking. The project will focus on young men aged 18-35. Men will be recruited via Match Day programmes, local Pompey Supporters Clubs, in the home fans tent (which is frequented by over 500 fans on match days) as well as local businesses, job centres and community events.

Middlesbrough

Middlesbrough will target 300 disadvantaged young men, aged 18 to 30 from across Tees Valley. In each year, 40 of these young men will have the opportunity to access an intensive individualised training course. The 'Live Well, Live safe!' project will provide these 300 young men with the opportunity to gain the advice and support they require to help achieve a healthy lifestyle - reduce their dependency on drugs, alcohol or cigarettes and support their progression into education, training or employment.

West Ham United

West Ham United will look to recruit men on match-days. West Ham United Community Trust stands will be located in the concourses, who will then provide access to the Tony Carr Men's Health Gym within Upton Park, where the men can register to be involved. They will then receive a prescriptive fitness programme.

An on-site clinical screening booth will also be built into the gym which will be used to monitor progress. The club also intends to work with the Bobby Moore Fund to help raise awareness of the programme as bowel cancer, which Bobby sadly died from aged just 51, is the second highest cause of death for men residing in Newham.

Newcastle United

Newcastle United Foundation will recruit two Health Trainers to work in the west end of Newcastle and Ashington. With a number of the club's children-focused community activities established in these areas, the Health Trainers will initially work alongside club coaches to engage with fathers who come along to support their children, but who do not maintain a healthy, active lifestyle themselves.

The Health Trainers and coaching staff will promote and run twice weekly football/multi sport sessions for the dads (and other men). The Health Trainers will also help increase health awareness and fitness levels, and address issues of alcohol misuse and obesity amongst the men.
